OK, I *think* I've finally solved this one.  Please download the latest version from the product page and upgrade your installation, and let me know if it fixes the problem. 

The reason it worked in some instances and not others is most likeyl due to Http Chunking, which is sending back chunks of html to the client as they are prepared.  Because of a bug in the module, replacements at certain points in the output html were not getting found/replaced.
