Thanks Bruce for your great support! Property preCondition="managedHandler" in <system.webServer> <modules> tag resolve problems. After restart IIS and load home Page I see in Log Viewer:

Well, I'm glad that is fixed.

Yes, this is a bug in the module. I recommend switching off the option to produce a duplicate page error in the log. You can safely ignore this error.
