Yes, that all looks OK to me.  Is it possible for you to try on a different IIS6 server, just to see if it might be something to do with the server itself?

Also, is there anything else in the httpModules list that might be taking precedence over the URl Rewrite entry and causing the 404? (wild guess)

Hey Bruce,

I almost forgot to update this post with the resolution. It seems I took for granted that the iFinity entry was added to the httpModules section when in fact it wasn't. I manually added the entry and everything started working quite well. I knew I was missing something obvious! I'm not quite sure why using URL Master correctly updated the providers section but not the httpModules section though.

Thanks for letting me know Ian.  I was almost going to say to check that earlier on, but then I've never heard of a failure to write the httpModules section before.  Next time I will just go with my instinct and say to check it as step 1.  Could have saved much messing about.
